About Me

My name is Brent Cunning, founder of DisruptionMSK and the Pain Coach In Your Pocket program.


I have a 4 year Honour's Specialization Bachelor's degree in Medical Cell Biology at the University of Western Ontario, a 2 year Master's degree in Physical Therapy from Queen's University, 2 years of additional training and certification in Mechanical Diagnosis and Therapy®. I was also formally trained as an advanced practice physical therapist.


That's just a fancy way of saying that I spent 3 years training with orthopaedic spine surgeons, neurosurgeons and other pain specialists in the province of Ontario, Canada, learning the appropriate criteria for imaging, cortisone injections, medications, or surgery for their pain.


I scrubbed into surgeries with the orthopaedic spine and neurosurgeons and even had my finger on someone's carotid artery (that's the pulse in your neck). Literally. Right on it. It was really cool. I learned how to interpret MRIs, CT scans and x-rays. And I learned the appropriate criteria for image-guided cortisone injections as well as the appropriate use for various pain medications.


I helped over 2500 people with back and leg pain figure out if they needed either exercises, imaging, injection, medication, or surgery for their pain. Family physicians relied on my opinion to determine how to properly manage their patient's with chronic low back pain.


I saw people across the spectrum of pain. From 4 weeks of minor acute low back strain to 40 years of chronic, debilitating, wheelchair bound leg pain and numbness. Some people did need surgery or specialty care. But the vast majority simply needed a healthcare provider who could empathize with them, give them effective exercises, and a lot of evidence-based education about pain.

Online Pain Coaching Program

1) Is This Physical Therapy?

Yes but it focuses more on physical diagnostics and understanding the problem. It uses physical movements, stretches and strengthening exercises that are considered physical therapy. But it's really a completely new paradigm for assessing and treating pain used by physical therapists, chiropractors, health coaches, doctors and surgeons all across the world. However, the pain coaching program can be billed as a physical therapy service in Ontario, Canada or in New York or Virginia since it's provided by a registered physical therapist in those provinces and states.

2) This Sounds Too Good To Be True. Why Haven't I Heard Of Rapidly Reversible Pain?

Because this rapidly reversible pain phenomenon is a relatively recent medical discovery. A physical therapist named Robin McKenzie first characterized this in 1956 (this is recent in terms of medical discoveries!). But the wheels of science turn slowly.

It has taken decades for the scientific community to recognize the existence of this problem and accept the MDT® methodology as the most reliable way to help people identify rapid reversible pain and treat it properly. There is now irrefutable scientific evidence to support this discovery that you can view here.

3) How Long Does It Take To Start Seeing Results?

Internal data from the quality-assurance data analytics organization we use, suggests that over 80% of individuals in pain have a rapidly reversible condition, meaning they can get significant improvement in symptoms within a few days with a single exercise. It's not always the same exercise for everyone, so it may take several visits to find which exercise is the "key" to unlocking rapid reversible pain.

4) Will This Be A Lot Of Work?

Depends on how you define a lot of work! For the first few weeks of the pain coaching program, we are seeing each other roughly every 48 hours. We'll guide you through simple movement tests online. You'll then have 1 exercise as homework for 48 hours until our next visit. Usually this is about 15-20 minutes total time commitment between visits which we can seamlessly integrate into busy work or travel schedules.

5) Will This Hurt?

Nothing you can't tolerate! We will never force you to push through pain unnecessarily. There are some conditions that do require "poking the bear" and creating a tolerable pain response, but we always provide clear and detailed instructions on how you can tell when it's time to stop an exercise or an activity.

6) Is This Covered By Benefits?

Probably not. Most individuals pay online using a credit card because most insurance plans do not cover the service. We don't bill insurance directly because often the insurance company bureaucracy creates more headache for the person in pain and the healthcare provider. It also creates a better bottom line for the insurance provider but may leave people without enough coverage, confused about what's causing their condition and still in pain after just a few covered sessions. Some healthcare spending accounts may cover it. You can submit the receipt to your insurance provider but we cannot guarantee that it will be covered. We do not see Worker's Compensation claims or Motor Vehicle Accident claims.

7) Can You Guarantee Results?

No. Nothing in healthcare is guaranteed. But we can guarantee you'll learn a lot about the cutting-edge science behind pain and some really effective strategies for resolving or managing pain yourself that you probably have never tried before. Even if you think you've done every exercise under the sun already. And we can guarantee you'll understanding your underlying condition and gain more confidence in being able manage it yourself or where you need to go if we're unable to help you completely resolve the problem.

8) What If My Pain Is Not Reversible?

The exercise testing process that we take you through will either reveal a rapidly reversible painful condition (with 1-2 simple exercises) or a slow reversible painful condition (with 1-2 simple exercises) in about 92% of cases. However, not everyone in pain will have a reversible condition with exercises. Internal data from the quality-assurance data analytics organization we use, suggests that about 8% of individuals in pain are in the "Other" category. This means their pain cannot predictably improve with exercises. That being said, many people in this category still get improvements through things like lifestyle interventions (breathwork, sleep, nutrition, hydration), time, general strength and conditioning, pain education and cardiovascular exercise. We can also reach out directly to your primary care provider if it seems like your issue could require medication, injection or surgical consultation.

9) Do I Need Special Equipment?

Nope. We'll use what you've got around your house. Usually all we need is a floor, a chair and a possibly a kitchen counter. If you've got exercise equipment, that's a bonus and we may use it. Sometimes we make recommendations to buy some cheap elastic resistance bands or lumbar cushions but it's not necessary prior to starting the program.
